Important Office Information

Thank you for choosing Shores Dental Center as your dental care provider. As new patients, we ask that you either come to the office ten (10) minutes prior to your dental appointment to fill out your new patient forms, or click the “New Patient” link below to fill out the forms, or if you would you like your new patient forms emailed to you, simply email us and let us know! We will promptly send you the forms via email. Print them out and complete them prior to your first visit. When you arrive at our dental office, you will only need to sign for your patient information that you filled out and go over your HIPPA information. Please be prepared to provide us a copy of your insurance card and picture ID.

New Patient Forms

Patients That Have Dental Insurance

We will be happy to file claims with your insurance carrier for you. We are not on any provider list and you will be responsible for any balance that your insurance company does not pay. Your insurance coverage is a contract between you or your employer and your insurance company, not with Shores Dental Center. The difference between our fee and the estimated insurance coverage is required at the time of service.

Shores Dental Center is not classified as a Medicaid provider. We are not able to file claims for Medicaid or Medicare.

Missed/confirmed Appointments

Unless canceled at least 48 hours in advance, our office policy is to charge for missed appointments at a rate of $50 per hour missed. Our office staff calls to verify your dental appointment(s), 4 days to 1 week before your scheduled appointment. An email is also sent to you. Please return their phone call(s) or leave a message on the office answering machine. We do need to know if you will or will not be able to come to your reserved appointment. Since we make every effort to contact/remind you of your upcoming appointment, we only respectfully require for you to confirm that you will be there. If you do not respond within the allotted time, we will unfortunately have to give your appointment day and time to another patient that is on the will call patient list and a charge for the missed appointment will be added to your account.

Financial Policy

We deliver the finest care at the most reasonable cost to our patients. A treatment plan is given to you at the time of your examination showing you an estimate. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Our office will file all necessary information to your insurance company.

We will send you monthly statements, if necessary. Most dental insurance companies will respond within four to six weeks. For patients with dental insurance, we will file your dental claims as a courtesy for you. Our office will check your dental claim status 45 days from the date of service. If your dental claim has not been paid within 60 days, you will receive a statement from us for the treatment rendered. You will have to contact your insurance company in order to be reimbursed. Any remaining balance after your insurance has paid it’s portion is your responsibility. Your prompt remittance is appreciated.

Payment Options

Option One: Pay in Full with Discount

We offer a 5% courtesy discount off the entire cost of treatment for all payments received in full by cash or debit card. We will gladly submit any insurance claim for you. However, your insurance is a contract between you and your insurance company.

Option Two: Installments

We accept Cash, Checks, and all major credit cards. We will go over your dental treatment with you and make your appointments according to what Dr. Shores suggests that needs to be done first. You will pay your estimated cost per procedure.

Option Three: CareCredit

We offer an outside billing source that offers free financing with no interest for up to 12 months and no money down for qualified patients. Our office has paid all the necessary fees for our patients to receive this offer interest free and it is one of our most popular. Please feel free to fill out an application at www.carecredit.com.

Option Four: Shores VIP Smile Club Program

We offer various membership levels to the Shores VIP Smile Club Programs. Each membership level carries a fixed monthly cost and gains you access to regular dental treatments. Learn more about the Shores VIP Smile Programs now!

We will be happy to accept insurance and estimate a co-pay based on your dental benefits and what your dental insurance offers. The co-payment we estimate for you is due at the time of your dental appointment and may be paid by cash, check, or credit card.
